Student Matinee Performance Registration Form

How to register for a Student Matinee Performance

Registering for a Student Matinee Performance is simple. Just review the instructions below and you’ll have what you need to complete the on-line registration. Included in the instructions is information on financial assistance, known as Subsidies, as well as the Participant Agreement.
  1. Select a performance
  2. Determine the number of student and adult tickets you will need.
    1. For student tickets, it is better to overestimate the number of tickets you will need as it is easier to cancel tickets than to add tickets to your order. Slight adjustments to final attendance numbers will be allowed up to the Participation Agreement deadline and is subject to availability. Tickets are $4 per student. Sales tax will be assessed if applicable.
    2. For adult tickets, student groups will receive one free adult ticket for every 15 student tickets. All adults who attend the performance must have a ticket, including teachers, chaperones, and bus drivers. Adult tickets may be purchased for $10. Please be sure to include all chaperones in your total chaperone count, even those with free tickets. Omaha Performing Arts reserves the right to limit chaperone tickets permitted by group.
  1. Financial assistance is available through our Subsidy Program, which is described in the second section of the form. Subsidies are available for assistance with tickets, transportation or both. Ticket subsidy is administered in the form of reduced ticket prices, and in some cases the ticket price is waived and the tickets are complimentary. The subsidy for bus transportation is made as a reimbursement check made out to the school or PTA’s/PTO’s after the performance occurs. To receive the bus subsidy reimbursement, you must provide a copy of the bus/transportation invoice after the performance. Estimates are not sufficient. All invoices and paperwork must be received no later than April 27, 2012 to receive reimbursement. Each school is responsible for full payment to the bus service provider. Bus subsidies can only be made payable to the schools, PTA’s or PTO’s.
  2. Once you are registered for the Student Matinee Performance, you will receive a Participant Agreement in the mail four to six weeks prior to the performance date. The Participant Agreement must be signed and returned with full payment to Omaha Performing Arts a full two weeks prior to the performance date. Those groups attending the event supported through the subsidy program must complete a Participant Agreement even if no payment is required

SUBSIDY SURVEY

Thanks to the generosity of The John K. and Lynne D. Boyer Foundation, Nebraska Arts Council, Mid-America Arts Alliance and the National Endowment for the Arts, Omaha Performing Arts is pleased to announce subsidy programs are available to help Omaha area school groups attend our Student Matinee Series performances.

Subsidies are offered to defray the costs of student tickets and bus transportation. They are distributed based on need and availability.

Both subsidies (Ticket Subsidy and Bus Reimbursement Subsidy) will be awarded on a percentage of total cost (25%-100%). Ticket subsidies will be administered in the form of reduced or waived ticket prices. Bus subsidies will be administered in the form of a reimbursement check AFTER the event. Each school is responsible for arranging their own transportation AND for full payment to the bus service provider.

In order to consider your subsidy request, we request answers to the following questions:
1. For which performance(s) are you requesting subsidy?
(This one completed survey is valid for any/all performance(s) for which you have registered.)

NOTE: All performances begin at 10:30am and are approximately 60 minutes in duration unless otherwise noted.
